terms & conditions

[est. 2017]

Person performing a shoulder stand exercise in the clinic.

These terms apply to appointments and services provided by Designed Wellbeing.

Cancellation policy

I understand that sometimes there is a need to cancel or reschedule an appointment, and I’m happy for you to change your appointment when needed. However, late cancellations and missed appointments affect my ability to offer that time to someone else.

For this reason, I ask for a minimum of 24 hours’ notice when cancelling or rescheduling an appointment.

A $50 late cancellation fee may apply where less than 24 hours’ notice is given.

A full appointment fee applies to appointments that are not attended.

The cancellation policy applies even if a courtesy reminder email or text has not been received from Designed Wellbeing.

In exceptional circumstances, I may choose to waive a cancellation fee at my discretion.

Payment

Payment is expected at the time of your appointment or within the timeframe specified on your invoice.

You are responsible for any private fees, ACC co-payments, or other charges associated with your treatment.

ACC coverage does not necessarily mean that there is no cost to you. If ACC declines cover, a service falls outside ACC funding criteria, or a private fee applies, the applicable amount remains your responsibility.

If an outstanding account requires referral to a debt recovery service, you will be responsible for reasonable recovery costs, including legal and court fees, to the extent permitted by law.

ACC

Where treatment is provided under an ACC claim, information required to manage and process that claim may be provided to ACC in accordance with the requirements of the ACC scheme and applicable privacy legislation.

You remain responsible for providing accurate information and notifying Designed Wellbeing of any changes to your ACC claim or circumstances that may affect funding.

Consent to assessment and treatment

Before treatment, I will explain the assessment or treatment I am proposing and answer any questions you may have.

By proceeding with an appointment, you consent to assessment and treatment by Designed Wellbeing. You have the right to ask questions, decline any part of your treatment, or withdraw your consent at any time.

You are also welcome to seek a second opinion or ask to be referred to another health professional where appropriate.

Communication

I may use your contact details to communicate with you about appointments, treatment, exercises, invoices and other matters relating to your care.

This may include communication by phone, email or text message.

While I take reasonable care when communicating electronically, electronic communication cannot be guaranteed to be completely secure. Please avoid sending highly sensitive information by email or text unless requested.
